Recommendation to use Foreign medical graduates to strengthen the Medical Health Infrastructure in India

Currently, there are more than 20,000 foreign medical graduates in India who want to help in the fight against Covid, but the government will not agree to allow them.

Foreign medical graduates in India have sent a letter to the government for a one-time exemption from the bridge exam that grants them a permit to practice in India.

The graduates who pursue their MBBS degree from countries like China, Ukraine, Russia, Bangladesh, the Philippines, Nepal, and Kazakhstan, have to clear the Foreign Medical Graduate Exam (FMGE)  to practice in India, as ordered by the MCI. However, the MBBS graduates from the leading countries, namely the US, UK, Australia, Canada, and New Zealand do not need to appear for this exam.

Thousands of Modern Medicine Trained foreign medical graduates (FMGs) of Indian origin are there who have completed their medical education from abroad, waiting to register in their country. Presently, these FMGs have to clear the MCI screening test. These FMGs also want to serve India, when the country is severely hit by the pandemic and is facing a huge crisis of shortage of doctors. Most of the exams in the country are getting canceled due to Covid-19 and the country is getting short of medical staff which was already below the population ratio. So, the FMGs are requesting that if they could get a one-time exemption from the exam and kept to the government compulsory contract, they will strengthen the healthcare system of the country.

There are possible solutions for this! as we stated before, Additional training and hands-on apprenticeships can be introduced to help FMGs build their skills to then be able to pass the FMGE. FMGs can learn by participating as observers in the established programs too. Apart from this, opportunities also exist for FMGs to work outside of clinical care, including research work, hospital administration, and public health. By that time, FMGs are an unutilized resource and lost opportunity to a country with shortages of physicians.

During a high-level conference, Dr. Rajesh Rajan, President of the Association of MD stated that - Almost every day we are losing doctors, including specialists. The existing teams are exhausted due to continuous work for the past year! The third wave will be more disastrous and medicos are needed to tackle it. There is a need to plan for the coming time.

He questioned that we have readymade doctors (the FMGs) then why is the government not allowing them to work? He added that the government has no issues if veterinary doctors are working with Covid teams but they are not ready to involve the FMGs in the same! He strongly recommended that FMGs must be added to the Indian Healthcare System to strengthen the healthcare infrastructure.

Hereby, most importantly we should review how the NMC has reacted to this recommendation and the requests of the FMGs.

Recently, The National Medical College had released draft regulations on Foreign Medical Graduates. According to that draft - in place of the Foreign Medical Graduate Exam (FMGE), the foreign medical graduates will be required to qualify for the National Exit Test (NEXT). The candidates aspiring to practice as a doctor in India will have to pass NEXT within two years of completing their MBBS program from abroad. Secondly, till the year 2020, the FMGE exam used to have only one-liner simple questions. The difficulty level of these questions was even less than NEET PG. But now according to the new draft, the students will be asked to increase the difficulty level of their preparation to five times. As per the information sourced, Every year around forty to fifty thousand graduates appear for the NEXT and their pass percentage is just 9-18%. The difficulty level of the exam can be analyzed well from this estimate!

The NEET result is valid for three years for MBBS abroad. Next, you are required to qualify for the NMAT exam. NMAT exam is organized in the Philippines in March and October. Bachelor's Degree or Diploma in the medical stream & allied sectors including Nursing,  Medical Lab Technician, B.Sc in Biology, BAMS, BUMS, biotechnology, etc, which is equivalent to a pre-medical course can apply for direct admission to the M.D. course with a duration of four years.
